How Families Apply for Child Care Assistance

The Child Care and Development Fund (CCDF) program provides financial assistance for families who are working or enrolled in school. To apply for the CCDF voucher program, you must contact your local Intake Agent.
PamMomm'E Child Kare is a CCDF eligible provider.
The information below is a partial list of what maybe needed. For the current/update requirements visit www.in.gov/fssa.
To be considered for the child Care voucher Program you must be currently working and/or attending school an/or, participating in an eligible IMPACT activity or have a referral from your DCS worker.
ID needed from you and your spouse (and/or child's father if applicable): Driver's License or State ID, Passport or one of the 4 Military, School, Work or Consulate ID.
All children in household one of the following: Birth Certtificate or Hospital Issued certificate of birth,Birth Confirmation letter, ICES screen, Immunization records, school records or State ID.
Proof of residency w/i 30 days: Lease, Utility bill, Mail, or valid Driver's Lic, Pay Ch stub, INS Green card or ICES Screen.
Hoosier Healthwise/Medicaid card for entire household if receiving Medicaid.
Foster Parents: All of the above plus: Valid Foster Parent License which matches the foster parent's residency verification and verification the child is ward of the State from the DCS caseworker or current per diem documentation with childre [ren] 's name on it.
Proof of a service need[working and/or attending school and/or participating in TANF/IMPACT Program.
If working [30 days prior to appointmentl][ 2 check stubs if paid bi-weekly 4 if paid weekly] 30 days of income or verification of hours worked, on company letter head or Employer ID number showing gross pay, hours worked, date paid, and employer's signature for most recent 30 days or employer must fill out Employment Hours/Wages Verification Form.
If starting new job: Verification of securing employment on company letterhead or including Employer ID Number, including date of hire, number of hours expected to work each week, hourly wage, employer's signature and date first paycheck will be received.

Provider Information Page completed by a licensed or certified CCDF provider only. A provider information page must be completed by your CCDF childcare provider before interview. *If you work for the child care provider where your children attend you must provide a statement signed by the Provider and yourself stating you do not take care of your own children.
